What is Medical Transcription? 

When a patient is seen by a doctor in his office, admitted to a hospital, seen in the emergency room, has an x-ray, or anytime there is patient interaction with a physician the doctors dictate a medical record. 

The medical transcriptionist then transcribes the medical information, converting it from dictated form into written form.  It is then sent hardcopy to the medical records department to be filed in the patient's chart.
